NEWS2 calculator
Written and checked against the NMC’s published sources by The NMCMATE editorial team.Last checked .
Enter a set of observations and this works out the NEWS2 score for each parameter and the total, along with the band the total falls in and whether any single parameter scored 3. It is the same arithmetic used in the OSCE simulator on this site, published here because it is useful on its own.
This is a calculator, not a clinical decision. It implements Scale 1 SpO2 only, which does not apply to a patient with hypercapnic respiratory failure, and the Royal College of Physicians chart is the authority on the scoring rather than this page. Use your employer’s own chart and escalation policy for anything involving a real patient.
Common questions
- What is a NEWS2 score?
- The National Early Warning Score 2 is a single number summarising six physiological observations plus whether the patient is on oxygen. Each is scored 0 to 3 against published thresholds and the scores are added. It is used across the NHS to standardise how a deteriorating patient is recognised and escalated.
- What does a single parameter scoring 3 mean?
- It escalates on its own, even when the total is otherwise low. A patient scoring 3 on one observation and nothing elsewhere has a total of 3, which is a low total, but the single 3 is what prompts urgent review. This calculator shows it separately for that reason.
- Does NEWS2 come up in the NMC Test of Competence?
- Recognising and escalating a deteriorating patient is part of the Future Nurse standards the clinical paper is built on, and observations feature in the OSCE. Knowing how the score is put together is more useful than memorising the table, because the chart is in front of you on the day.
- Why does this only do Scale 1?
- Scale 2 applies to patients with hypercapnic respiratory failure, where a lower oxygen saturation is the target, and using it requires a clinical decision that has already been made and recorded for that patient. A calculator cannot make that decision, so offering the scale would invite the wrong one to be picked.
